Candidates using Appendix-D should discard the earlier format and use only the revised document now available on the MCC website.
The Medical Counselling Committee has updated its official notice for PwBD candidates and Medical Assessment Boards following a corrigendum issued by the National Medical Commission’s Undergraduate Medical Education Board.
The main PwBD eligibility-certificate rules remain applicable, but the affidavit format under Appendix-D has been corrected.
What Has Changed?
The updated MCC notice specifically incorporates a revised:
Appendix-D — Locomotor Disability Affidavit
The corrected affidavit applies to candidates whose locomotor disability assessment involves lower-extremity stability components.
The revised self-declaration asks the candidate to declare functional ability relating to activities such as:
- Bearing weight and standing on both legs
- Bearing weight and standing on the affected leg
- Walking on plain surfaces
- Sitting independently on a chair
- Turning to the right and left
- Use or non-use of an assistive device or artificial limb
The affidavit also includes declarations concerning:
- The candidate’s diagnosed disability
- Functional ability
- Capacity to undertake the MBBS course
- Consequences of submitting a false declaration
- Candidate signature
- Date and place
- Notarisation
⚠️ The revised Appendix-D is marked as corrected through the UGMEB, NMC corrigendum dated 31 July 2026.

🚨 Any HGE alert, WhatsApp message, document checklist or downloadable form linking to the earlier PwBD PDF should be updated immediately.
Who Is Affected?
This correction is particularly important for NEET UG 2026 candidates who:
- Are applying under the PwBD category
- Have a locomotor disability
- Are required to submit Appendix-D
- Are visiting an MCC-designated Medical Assessment Board
- Have already downloaded or prepared the previous Appendix-D affidavit
Candidates using other disability-specific appendices should still use the complete updated MCC notice and verify that they are submitting the affidavit applicable to their disability category.
What Has Not Changed?
The following core PwBD rules remain unchanged:
- Candidates claiming PwBD reservation must obtain an Eligibility Certificate from a designated Medical Assessment Board.
- A permanent UDID card is required.
- Candidates must submit the prescribed functional-ability affidavit.
- Candidates must submit Appendix-A and the applicable disability-specific affidavit.
- Only candidates with the MCC Eligibility Certificate will be shown PwBD seat choices.
- The certificate is valid for the 2026–27 academic session only.
- Fake UDID cards, affidavits or supporting documents may lead to seat cancellation and legal action.
Action Required for Candidates
- Delete or stop using the earlier Appendix-D format.
- Download the revised MCC PwBD notice.
- Locate the corrected Appendix-D in the updated PDF.
- Complete the affidavit carefully and truthfully.
- Do not alter the questions or prescribed declaration.
- Get the affidavit notarised where required.
- Carry it with:
- NEET UG 2026 scorecard
- Permanent UDID card
- Appendix-A
- Relevant medical records
- Assistive-device details
- Other prescribed documents
- Confirm that the selected MCC assessment centre evaluates the candidate’s disability type.
Important Warning
Submitting the old Appendix-D after an official corrigendum may create a document-verification issue or require the candidate to submit the corrected affidavit again.
Candidates should not rely on:
- An older downloaded copy
- Screenshots shared in groups
- Unofficially typed affidavit formats
- Previous-year PwBD affidavits
- Affidavits prepared without checking the revised MCC document
No Separate Deadline Announced
MCC has not mentioned a separate last date for using the revised Appendix-D in this notice.
PwBD candidates should complete the medical assessment and certificate process as early as possible before MCC choice filling.
Do not wait until the final days, because assessment may require multiple specialists, additional tests and document corrections.
